In fact, you may look more swollen on Day 3 than you did on Day 1.

Understanding what usually happens during the first two weeks can make recovery much less stressful.

This guide explains the typical early healing process after procedures such as facelift, endoscopic forehead and brow lift, upper or lower blepharoplasty and combined facial surgery.

Every recovery is individual. Your surgeon’s instructions always take priority.

The first thing to know: healing is not linear

One of the most common concerns after facial surgery is:

“Why do I look worse today than yesterday?”

Because postoperative swelling does not disappear immediately after surgery.

During the first 48–72 hours, the body’s inflammatory response is still developing. Fluid accumulates within the tissues, bruising becomes more visible and swelling may continue to increase.

For many patients, Days 3–4 can be the most swollen stage of early recovery.

This means that looking more swollen on Day 3 than on Day 1 does not necessarily mean something is wrong.

And most importantly:

The face you see during the first few days after surgery is not your surgical result.

Day 1: The beginning of recovery

During the first hours after surgery, you may feel sleepy, tired or slightly disoriented following anaesthesia.

Your face may feel:

tight;

heavy;

numb in some areas;

tender;

mildly swollen.

Interestingly, immediately after surgery you may not yet look as swollen as you expected.

This is because postoperative oedema continues to develop over the following hours.

You may also notice small amounts of blood staining on dressings or surgical tapes. Your surgical team will advise what is expected for your particular procedure.

What matters on Day 1?

Rest.

Follow your surgeon’s instructions regarding sleeping position, dressings, medication and movement.

After many procedures, gentle mobilisation is encouraged when medically appropriate.

Do not manipulate, massage or remove dressings from the operated area unless specifically instructed to do so.

Day 2: Swelling becomes more visible

On the second day, swelling and bruising may become considerably more noticeable.

After forehead, brow or eyelid surgery, you may notice swelling around:

the forehead;

temples;

upper eyelids;

lower eyelids;

cheeks.

The eyes may begin to look narrower because of the surrounding oedema.

One side may look different from the other.

This early asymmetry can be unsettling, but tissues do not necessarily swell identically on both sides.

Is it normal to look worse on Day 2?

It can be.

Your body is still in the active inflammatory phase of healing.

Do not start evaluating symmetry or aesthetic results at this stage.

Day 3: Often the peak of swelling

Day 3 is particularly important psychologically.

Many patients expect to look progressively better every day after surgery. Instead, they may wake up on the third morning looking significantly more swollen.

For many facial surgery patients, this is around the time when oedema is at or close to its maximum.

Your face may look:

very puffy;

temporarily asymmetrical;

bruised;

unfamiliar;

dramatically different from your normal appearance.

Your eyelids may be particularly swollen and your eyes may appear much smaller.

Some patients describe looking almost unrecognisable.

Is this my result?

No.

At this stage you are seeing postoperative anatomy plus swelling, inflammation and bruising.

You are not seeing the final position or contour of healed tissues.

Day 3 is therefore one of the worst possible moments to judge whether you like the result of facial surgery.

Day 4: Swelling may still be significant

Do not expect everything to suddenly improve after the third day.

Day 4 may still look very similar.

For some patients, this is still part of the peak swelling period.

However, if recovery is progressing normally, the swelling should gradually stop increasing.

Bruising may also begin changing colour.

What should you watch?

The overall direction of recovery matters more than one photograph.

A face that remains swollen is very different from a sudden, rapidly increasing swelling, particularly if it occurs predominantly on one side or is associated with increasing pain.

If something changes suddenly, contact your surgical team.

Day 5: The first turning point

Around Days 4–5, many patients begin to notice the first signs that swelling is stabilising.

The change may be subtle.

You may notice that:

your eyes open a little more easily;

facial pressure feels reduced;

swelling feels slightly softer;

bruising begins to change colour.

Do not expect your normal face back yet.

This is still very early healing.

Days 6–7: Beginning to recognise yourself again

By the end of the first week, many patients begin to feel psychologically better because the face starts looking more familiar.

Swelling should generally be moving in the right direction, although it may still be very noticeable.

Bruising may become yellow, green or brown as blood pigments are gradually broken down.

Because of gravity, swelling and bruising may also appear to move lower on the face.

This does not necessarily mean that the condition is spreading or becoming worse.

Week 2: Visible improvement, but not the final result

During the second week, the most dramatic early postoperative appearance usually continues to improve.

You may begin feeling more comfortable seeing other people.

However, you may still notice:

puffiness around the eyes;

forehead or temple swelling;

facial tightness;

numbness or altered sensation;

mild asymmetry;

firmness within healing tissues.

All of these can persist beyond two weeks depending on the procedure.

Should my face look completely normal after two weeks?

Not necessarily.

Two weeks is a recovery milestone — not the end of healing.

Residual swelling after facial surgery can continue decreasing over several weeks and, following more extensive procedures, tissue settling continues for months.

Why does swelling sometimes look worse in the morning?

Postoperative swelling can fluctuate during the day.

After spending several hours lying down overnight, fluid distribution can make the face appear puffier in the morning.

Your appearance may therefore change between morning and evening.

This is one reason why comparing photographs taken at different times of day can be misleading.

Why is one side more swollen than the other?

The human face is not perfectly symmetrical before surgery, and the two sides do not necessarily respond identically afterwards.

Differences can occur because of:

individual anatomy;

tissue characteristics;

bruising;

fluid distribution;

sleeping position;

differences in the extent of surgical correction.

Mild asymmetry during early recovery is therefore common.

However, new or rapidly increasing one-sided swelling, especially when accompanied by increasing pain or bleeding, should be assessed by your surgical team.

What can I do to help swelling?

Your individual postoperative instructions should always come first.

Depending on your procedure, your surgical team may recommend:

Keep your head elevated
After many facial procedures, sleeping with the head and upper body slightly elevated during early recovery can help manage swelling.

Keep comfortably cool
Avoid excessive heat during early healing.
Hot baths, saunas and other significant heat exposure should wait until your surgical team says they are appropriate.

Walk gently
Recovery does not necessarily mean remaining in bed.
When medically appropriate, short, gentle walks support circulation and help avoid prolonged immobility.
Move slowly. Rest frequently. Do not turn walking into exercise.

Do not massage your face yourself
Do not attempt to push swelling away.
After facial surgery, tissues need time to stabilise. Massage, lymphatic drainage or other manual therapy should only be performed when appropriate for the procedure and approved by your surgical team.

Follow your postoperative care plan
Dressings, surgical tapes, wound care, medication and follow-up appointments all have specific purposes.
Do not remove or modify them simply because swelling looks dramatic.

Lymphatic drainage after facial surgery

Postoperative lymphatic care should be individualised.

Early treatment does not necessarily mean directly massaging recently operated facial tissues.

Depending on the procedure and stage of healing, postoperative care may focus on appropriate lymphatic pathways such as the neck and décolletage, while avoiding unnecessary manipulation of newly operated areas.

The timing and technique should be determined by the surgical and postoperative care team.

At Clinica Etica, we adjust postoperative care according to what we see during recovery rather than applying an identical protocol to every patient.

Photobiomodulation during recovery

When clinically appropriate, photobiomodulation may form part of a personalised postoperative recovery programme.

Its use, timing and treatment parameters should be selected according to the procedure and individual healing process.

It does not replace medical assessment, wound care or surgical follow-up.

When should I contact my surgeon?

Most swelling and bruising during the first week are expected.

However, contact your surgical team promptly if you experience:

sudden or rapidly increasing swelling;

significant new swelling predominantly on one side;

severe or increasing pain;

persistent fresh bleeding;

increasing redness, heat or abnormal wound discharge;

fever or feeling significantly unwell;

new visual changes after surgery around the eyes;

marked pain or swelling in one leg.

Shortness of breath, chest pain or sudden significant breathing difficulty require urgent medical assessment.

When in doubt, contact your surgical team rather than trying to diagnose the problem yourself.

<h2
